Çalışan: 员工全名, T.C. kimlik no: 身份证号

入职日期 – 结束日期 tarihleri arasında yukarıdaki işverende çalıştım. İş sözleşmem 结束日期 tarihinde sona ermiştir.

Aşağıda türü ve tutarı yazılı alacaklarımın tamamı 付款日 tarihinde 银行名称 bankasındaki hesabıma ödenmiştir:

Kıdem tazminatı: 遣散费（税后，TRY） TL
İhbar tazminatı: 预告补偿（税后，TRY） TL
Kullanılmayan yıllık izin ücreti: 未休年假（税后，TRY） TL
Son ay ücreti: 最后一个月工资（税后，TRY） TL

When is it used?

After the employment has ended and all claims have been paid, to document that the employee releases the employer from those claims.

Validity conditions

  1. It must be in writing.
  2. It must be made at least one month after the contract ends.
  3. The type and amount of each released claim must be stated clearly.
  4. Payment must be full and made through a bank.

A release signed on the day of termination is absolutely void. A document showing partial payment is a receipt, not a release. A safer route is a mediation settlement: Mutual termination and releases
